University of Montreal Hospital Center (CHUM)
The Centre d’expertise en neurofibromatose at the CHUM is an interdisciplinary clinic offering specialized care for adult patients with neurofibromatosis type 1 (NF1), schwannomatosis (SWN) or NF2-related schwannomatosis (NF2-SWN). It was inaugurated in 2021 by Dr. Sarah Lapointe, Dr. Moujahed Labidi and Dr. Zaki El Haffaf.
It is the only clinic in Québec and Eastern Canada to offer comprehensive patient care. Its team includes a neuro-oncologist (Dr. Sarah Lapointe), a neurosurgeon (Dr. Moujahed Labidi), a genetic physician (Dr. Zaki El Haffaf), an endocrinologist (Dr. Isabelle Bourdeau), a genetic counsellor (Danny Tremblay) and a nurse clinician (Emma de Haan).
Care is coordinated to facilitate patient follow-up. Members of the Centre d’expertise work closely with specialists in psychiatry, neuropsychology, ophthalmology, dermatology, plastic surgery, orthopedics, internal medicine and endocrinology. In addition to the medical examination, particular attention is paid to the prevention and treatment of complications.
It’s important to know that to be followed up at the Centre d’expertise en neurofibromatose du CHUM, you absolutely must have a referral or a request from a healthcare professional. If you don’t have a family doctor, or if your doctor doesn’t know where to refer you, you can provide him or her with the “Demande de consultation, Neurologie – Centre d’expertise en neurofibromatose” form, which can be found here. Here’s how to send your request:
- Obtain the consultation request form.
- Advise your family physician to complete the application and fax it to 514-412-7000.
- If you don’t have a family doctor, you must make an appointment at a walk-in clinic, ask your doctor to fill out the request form, and fax it to the CHUM.
- Your request will be triaged by the NF clinic nurse, and depending on priority, patients are usually seen after one to 12 months.
For patients already being treated by the CHUM NF clinic who wish to have a follow-up or simply ask questions, please call 514-890-8123. It is very important NOT to make a new consultation request.
